Instructions for Using Generic Application When you have read these instruction press the submit button and after you print each page press the continue button until you have printed the entire application. Fill in the application and fax to our office for processing. Use a separate application for each Life Insured, except for Children's Term Rider's and Owner's Waiver of Premium. Do not use the generic application for a Trial Application. Print clearly with pen in black or blue ink only.
The full name of the company must be indicated in question 1 a). Do not use short forms or acronyms.
Definition of Non-Smoker
Joint Life refers to a policy where the basic Sum Insured covers more than one life but the death benefit is paid only once on one death. The OWNER section must be completed if the owner is other than the proposed life insured or if Waiver of Premium is applied for on the owner. All questions not applicable should say n/a to indicate the question has been considered. Not all companies offer RRSP life products. When completing question 7 a) ensure that you have confirmed the specific company's guidelines. When questionnaires are required, use the appropriate questionnaires e.g. avocation, aviation, chest pain, etc. supplied by the specific company. If a variable life insurance policy has been selected, ensure that the appropriate Information Folder has been left with the Prospective Contract holder/owner and that he/she completes and signs the Acknowledgment of Receipt of Information Folder in this application. Ensure that the Owner has read and understands the section "Declarations, Agreement, Authorizations and Signatures". Every application must be signed, witnessed and dated the day the application was completed.
